Why progesterone is tested

Progesterone is a hormone of the second phase of the menstrual cycle, produced by the corpus luteum after ovulation and by the placenta during pregnancy. It prepares the lining of the womb for embryo implantation and maintains pregnancy. The test answers three questions: whether ovulation has occurred, whether the second phase of the cycle is functioning adequately, and how an early pregnancy is progressing.

When to consider this test

  • difficulty getting pregnant, suspected anovulatory cycles, irregular periods,
  • investigation of luteal phase deficiency and recurrent miscarriage,
  • early pregnancy: assessment of prognosis, suspected ectopic pregnancy (always alongside beta-hCG),
  • abnormal uterine bleeding, suspected PCOS,
  • monitoring progesterone treatment or ovulation induction.

How to prepare

  • You do not need to fast. Drink a glass of water before your blood sample is taken.
  • Women who menstruate: have the sample taken on days 21–23 of a 28-day cycle or 7 days before your expected period; if your cycles are irregular, your doctor will decide on the timing, and the test may sometimes be repeated every 2–3 days.
  • Repeat the test at the same time of day; the level can fluctuate by as much as half within a few hours.
  • Stop taking high-dose biotin 12 hours before your blood sample is taken. Tell your doctor about contraception, HTZ (hormone replacement therapy) and progesterone preparations (including vaginal preparations).

How to interpret your result

The result is reported in ng/ml or nmol/l (1 ng/ml = 3,18 nmol/l). Interpretation depends on the phase of your cycle or week of pregnancy; laboratory reference ranges vary, so compare your result with the ranges on your report. As a guide (Mayo Clinic Laboratories):

Situation Reference range
follicular phase (before ovulation) up to 0,89 ng/ml
luteal phase (after ovulation) 1,8–24 ng/ml; above 3 ng/ml confirms ovulation
trimester I of pregnancy 11–44 ng/ml
trimester II 25–83 ng/ml
trimester III 58–214 ng/ml
after menopause, men up to 0,2 ng/ml

A low result in the second phase indicates a lack of ovulation or inadequate corpus luteum function; causes include PCOS, thyroid disorders, hyperprolactinaemia, stress, malnutrition and perimenopause. In early pregnancy, a single result does not determine the prognosis; your doctor assesses it alongside beta-hCG and USG (ultrasound).

What to test alongside progesterone

For fertility investigations: FSH and LH (days 3–5 of the cycle), oestradiol, prolactin, TSH, AMH; during pregnancy, beta-hCG. Convenient testing with a single blood sample: women’s hormone test package. Find out more: Badania-Medyczne.pl.

Frequently asked questions

On which day of the cycle should progesterone be tested?

In a 28-day cycle, on days 21–23, approximately 7 days after ovulation and 7 days before your expected period. For cycles of a different length, count back 7 days from your expected period. A test taken on the wrong day gives a “low” result despite normal ovulation.

What result confirms ovulation?

Progesterone above 3 ng/ml (approximately 10 nmol/l) in the second phase of the cycle usually confirms that ovulation has occurred. Peak levels in the middle of the luteal phase are considerably higher.

Do I need to fast?

No. Progesterone levels are not affected by meals. The day of your cycle and testing at the same time of day when repeating the test are more important.
