PSA – A basic test for assessing the risk of prostate enlargement or prostate cancer.
Description
Why PSA is tested
PSA (prostate-specific antigen) is a protein produced by the prostate gland, a small amount of which enters the blood. The test measures total PSA, which is the sum of free PSA and PSA bound to proteins. Levels rise with age and in benign prostate enlargement, prostatitis and prostate cancer. PSA helps guide decisions about further investigations for prostate cancer, but the result alone does not establish a diagnosis: only a biopsy can confirm cancer. The test is also used to monitor men after treatment for prostate cancer.
When to consider this test
- as part of early detection of prostate cancer, after discussing it with a doctor (the European Association of Urology recommends an initial test at the age of 40–45),
- if you have difficulty passing urine, a weak urine stream or frequent urination at night,
- if you have blood in your urine or pain in the perineal or pelvic area,
- for monitoring after treatment for prostate cancer (surgery, radiotherapy),
- to follow up a previous raised result.
How to prepare
- For 48 hours before blood sampling, avoid ejaculation and strenuous exercise, especially cycling, which puts pressure on the prostate.
- Have blood taken before a digital rectal examination, prostate biopsy, cystoscopy or catheterisation; if these have already taken place, wait 2–3 weeks.
- Tell your doctor about any medicines you take: some medicines used for prostate enlargement lower PSA and affect how the result is interpreted. Do not stop taking them without medical advice.
Understanding your result
| Result | What it usually means |
|---|---|
| Within the range for your age | a low likelihood of significant prostate disease, but does not rule out cancer |
| Above the range for your age, usually above approximately 4 ng/ml | requires assessment by a urologist: common causes include prostate enlargement or prostatitis, but cancer is also possible; the test is often repeated and free PSA measured |
| Above 10 ng/ml | always requires further investigations for prostate cancer |
| After prostate removal: a result above 0,2 ng/ml | may indicate recurrence of the disease; decisions are made by a urologist or oncologist |
The normal range depends on age: the upper limit is lower in younger men and higher in older men. A normal PSA level does not rule out prostate cancer, and a raised level does not automatically mean cancer. Use the reference range provided on the laboratory report.
Other tests to consider alongside PSA
Free PSA, Urinalysis, Testosterone, Creatinine, Full blood count. More information: Badania-Medyczne.pl.
Frequently asked questions
Does a raised PSA level mean prostate cancer?
Not always. The most common cause of raised PSA is benign prostate enlargement. Prostatitis, urinary tract infection, ejaculation, cycling and urological procedures can also raise the result. A urologist decides on the next steps, such as repeating the test, measuring free PSA or performing a biopsy.
How many days before a PSA test should I avoid sex and cycling?
The laboratory recommendation is to avoid ejaculation and strenuous physical activity, including cycling, for at least 48 hours. After a prostate biopsy or digital rectal examination, a wait of 2–3 weeks is recommended.
What is the difference between total PSA and free PSA?
Total PSA includes all PSA in the blood, whereas free PSA includes only the portion not bound to proteins. When total PSA is raised, a low percentage of free PSA increases the likelihood of cancer. Free PSA is usually measured as an additional test.

How we verify reviews
Reviews of tests can only be added by people who bought the test at BadamySie.pl. A review can only be added via the link in the e-mail sent after the order is completed, so every review is linked to an order. Our team checks text comments before they are published. We also publish negative reviews, without changing their content; we only reject spam, offensive language and personal data. The stars above the test description (“Rating: … · … ratings”) are votes from website visitors and are not verified.
