Why Borrelia antibodies are tested
This panel is the first stage of Lyme disease investigations: measuring IgM antibodies (which appear earlier, during active infection) and IgG antibodies (which appear later and persist for a long time) using ELISA or CLIA. These tests are sensitive but have low specificity: they cross-react with antibodies produced in other infections and autoimmune diseases. Diagnosis therefore involves two stages: a positive or equivocal result requires confirmation by Western blot (immunoblot), and the doctor makes the diagnosis based on symptoms, not the result alone.
When to consider this test
- 3–4 weeks or more after a tick bite if symptoms develop: an atypical red rash, fever, joint and muscle pain or tiredness,
- recurrent or migratory pain and swelling in large joints (especially the knee) without injury,
- facial nerve palsy, nerve root pain, numbness or meningitis (neuroborreliosis; the doctor extends investigations to include cerebrospinal fluid),
- heart rhythm disturbances or atrioventricular block in a young person after spending time in woodland,
- chronic skin changes on the limbs (atrophic dermatitis).
How to prepare
- Fasting is not required. Wait at least 3–4 weeks after the bite; otherwise, the result will be negative despite infection.
- If you have typical erythema migrans, see a doctor rather than attending a laboratory: treatment starts immediately.
- Tell your doctor about immunosuppressive medicines, previous syphilis, glandular fever or autoimmune diseases: these may cause false-positive results.
- Testing the tick for bacteria is not useful; a positive result in the tick does not mean the person is infected.
Understanding your result
Results are reported as negative, equivocal (borderline) or positive, usually with a numerical value in units specified by the test manufacturer.

IgG antibodies may persist for years after successful treatment, so they are not used to confirm recovery. Diagnosing Lyme disease requires clinical symptoms plus two positive tests (ELISA and Western blot).
Other tests to consider
After a positive or equivocal result: IgG and IgM Western blot. For joint pain, the doctor also requests CRP, ESR, full blood count, rheumatoid factor; after a tick bite in endemic areas, also tick-borne encephalitis antibodies. Read more: Badania-Medyczne.pl.
Frequently asked questions
When should I have a Lyme disease test after a tick bite?
No earlier than 3–4 weeks after the bite. IgM antibodies appear after 2–4 weeks, and IgG after 4–6. Testing immediately after removing the tick gives a false-negative result and is not useful.
I have a red rash after a tick bite. Should I have a test?
No. Typical erythema migrans (an expanding patch larger than 5 cm, often with central clearing, appearing after a few days to a few weeks) is a basis for immediate antibiotic treatment without testing; antibodies are usually not yet present at this stage.
My result is positive. Do I have Lyme disease?
This is not yet certain. ELISA is the first stage; a positive or equivocal result must be confirmed by Western blot. IgG may remain positive for years after a previous, successfully treated infection, and IgM can be falsely positive.
