What this package assesses
Two hormones that together indicate the stage of ovarian function. Oestradiol is the main oestrogen produced by ovarian follicles; follicle-stimulating hormone (FSH) is a pituitary hormone that stimulates these follicles. As ovarian function declines, oestradiol falls and the pituitary responds by producing increasingly higher levels of FSH. High FSH with low oestradiol is the most typical menopausal pattern; this package helps distinguish menopause from other causes of absent periods and hot flushes (thyroid disorders, hyperprolactinaemia, stress and malnutrition).
When to consider these tests
- Irregular or absent periods after age 40, hot flushes, night sweats, insomnia, mood changes, vaginal dryness or reduced libido,
- Suspected premature ovarian insufficiency (symptoms before age 40) or early menopause (before 45),
- Before starting menopausal hormone therapy and when assessing whether contraception can be stopped,
- After a hysterectomy (the absence of periods makes it impossible to assess the stage),
- During infertility investigations: assessment of ovarian reserve alongside anti-Müllerian hormone (AMH).
How to prepare
- Attend in the morning after a full night’s sleep; fasting is not required, but avoid a heavy meal and exercise the day before.
- If you still have periods: on days 2–5 of the cycle. If not: on any day, with the possibility of repeat testing.
- Hormonal contraception and hormone replacement therapy (HTZ) interfere with the results; your doctor decides whether they should be stopped.
- Stop taking high-dose biotin for 2–3 days before the tests.
How to read the results
Follicle-stimulating hormone (FSH) is reported in IU/l (mIU/ml), and oestradiol in pg/ml or pmol/l (1 pg/ml = 3,67 pmol/l). Approximate FSH ranges according to Mayo Clinic Laboratories: follicular phase 2,9–14,6, after menopause 16–157 IU/l, typically above 40 IU/l.

Menopause is diagnosed clinically: 12 months without a period. Hormone tests support the clinical picture and are most useful in younger women, after a hysterectomy or when symptoms are atypical.

What FSH level indicates menopause?
After menopause, FSH is usually above 40 IU/l and oestradiol is low. A single result is not enough: during the transition, FSH fluctuates from month to month, so a doctor will often repeat the test after 4–6 weeks.
On which day of the cycle should FSH and oestradiol be tested?
If you still have periods: on days 2–5 of the cycle. If periods are irregular or have stopped, the day does not matter, but the tests may need to be repeated.
Are results reliable while using hormonal contraception?
No. Pills, patches and vaginal rings suppress FSH and alter oestradiol; the results do not reflect ovarian function. Agree with your doctor when to stop them before testing.
