Why a lipid profile is tested

A lipid profile measures four components in one sample: total cholesterol, LDL cholesterol (so-called bad cholesterol), HDL cholesterol (so-called good cholesterol) and triglycerides; many laboratories also calculate non-HDL cholesterol. It is a basic test for assessing the risk of atherosclerosis, heart attack and stroke. Lipid disorders cause no pain and may remain symptom-free for years, yet they are among the strongest and most readily modifiable risk factors.

When to consider this test

  • as a preventive check every 5 years from age 20, and more frequently after age 40,
  • annually if you are overweight, have diabetes, high blood pressure or chronic kidney disease, smoke or lead a sedentary lifestyle,
  • if a family member had a heart attack or stroke before age 55 (men) or 60 (women), or very high cholesterol (suspected familial hypercholesterolaemia),
  • if you have yellow deposits on the eyelids or tendons, or a history of pancreatitis,
  • during statin treatment and after dietary changes, to assess their effect.

How to prepare

  • Fasting is not required. If your doctor requests a fasting sample or your previous triglyceride result was high, do not eat for 8–12 hours.
  • Avoid alcohol for 48–72 hours (it raises triglycerides), fatty meals for 4 hours and unusually strenuous exercise the day before.
  • Avoid testing during an acute infection or within a few weeks of a heart attack, surgery or serious illness; the result will be lower than usual.
  • Maintain your usual diet for 2–3 weeks before the test; a single day of “healthy” eating makes no difference.

Understanding your result

Results are reported in mg/dl (sometimes mmol/l; cholesterol: 1 mmol/l = 38,7 mg/dl). There is no universal normal LDL level; the target depends on risk, which the doctor assesses using age, blood pressure, smoking, diabetes and vascular disease. Targets according to the ESC/EAS guidelines from 2019:

very high below 55 mg/dl (1,4 mmol/l) previous heart attack or stroke, plaques in the arteries, diabetes with complications

Triglycerides below 150 mg/dl indicate lower risk; above 400 mg/dl, the laboratory formula for LDL no longer works and the test is repeated fasting. High HDL is protective, but does not by itself “cancel out” high LDL. The results are assessed together, alongside blood pressure, glucose and medical history.

Other tests to consider alongside a lipid profile

For a complete risk assessment, the doctor also requests glucose or HbA1c, creatinine, TSH (an underactive thyroid raises cholesterol), liver function tests before starting a statin, and lipoprotein (a) once in a lifetime. Read more about cholesterol and diet: Badania-Medyczne.pl.

Frequently asked questions

Do I need to fast for a lipid profile?

According to European guidelines, fasting is not required for risk assessment. After a meal, triglycerides are higher by an average of about 27 mg/dl, which is not significant for most people. If triglycerides are above 400 mg/dl, the test is repeated fasting.

What is a normal LDL cholesterol level?

There is no single normal level. The target depends on cardiovascular risk: below 116 mg/dl for low risk, below 100 for moderate risk, below 70 for high risk and below 55 mg/dl for very high risk (e.g. after a heart attack).

How often should I repeat a lipid profile?

Healthy people aged 20 and over: every 5 years; people who are overweight, have diabetes, high blood pressure or a relevant family history, or smoke: annually; during statin treatment: as advised by the doctor.
