Why vitamin D is tested

The test measures the concentration of 25-hydroxyvitamin D (calcidiol, 25(OH)D), the form in which vitamin D circulates in the blood and which best reflects its stores from skin synthesis, diet and supplements. Vitamin D supports calcium and phosphate absorption, bone mineralisation and muscle function; deficiency causes rickets in children and osteomalacia and osteoporosis in adults, and is also associated with reduced immunity and persistent tiredness.

In Poland, deficiency is common from October to April because the angle of sunlight prevents synthesis in the skin. The result helps determine a suitable supplement dose instead of taking one without knowing your level.

When to consider this test

  • before starting supplementation and around 3 months afterwards, then once a year,
  • for bone and muscle pain, weakness, frequent infections or fractures after minor injury,
  • in people who are overweight or obese, pregnant, over 65 or have little sun exposure,
  • in kidney and liver disease, malabsorption disorders (coeliac disease, Crohn’s disease) or osteoporosis,
  • during long-term treatment with glucocorticoids, antiepileptic medicines or immunosuppressants.

Polish guidelines do not recommend testing the whole population; testing is strongly recommended for people in risk groups.

How to prepare

  • Fasting is not required. Drink a glass of water around 30 minutes before sample collection.
  • If you take vitamin D supplements, do not stop them before the test; note the dose and tell your doctor when discussing the result.
  • If you take high-dose biotin (hair and nail supplements), stop it for one day before the test; it may distort results obtained using immunoassays.

Understanding your result

Results are reported in ng/ml (sometimes in nmol/l; 1 ng/ml = 2,5 nmol/l). Thresholds according to the Polish guidelines from 2023:

A low result alone is not a disease, but a signal to correct the deficiency. If the result is above 100 ng/ml, the doctor checks calcium, as this determines the symptoms of toxicity.

Other tests to consider alongside vitamin D

Frequently asked questions

Do I need to fast for a vitamin D test?

No. You can eat a light meal. The result does not depend on the time of day, although morning blood collection is usually most convenient.

What is a normal vitamin D result?

According to the Polish guidelines from 2023, the optimal 25(OH)D concentration is 30–50 ng/ml. A result of 20 ng/ml or below indicates deficiency, 20–30 ng/ml a suboptimal level and above 100 ng/ml a risk of toxicity.

When should I repeat the test after starting supplementation?

For people aged 11 and over, repeat testing is performed after 8–12 weeks, no later than 3 months. Once the dose has been established, annual testing is sufficient.
