The most important pathogen is group A streptococcus (<em>Streptococcus pyogenes<\/em>), which causes 15\u201330% of cases of pharyngitis in children and a few per cent in adults, and requires antibiotic treatment because of the risk of complications. The remaining 80\u201390% of cases of pharyngitis are caused by viruses, which a culture cannot detect and antibiotics cannot treat; the test therefore enables targeted treatment rather than treating blindly.<\/p>\n<h2>When to consider this test<\/h2>\n<ul>\n<li>fever, severe throat pain when swallowing, patches on the tonsils, enlarged and tender lymph nodes in the neck, without a runny nose or cough (signs of bacterial tonsillitis),<\/li>\n<li>recurrent tonsillitis, no improvement after antibiotics, suspected carriage within the family,<\/li>\n<li>before deciding whether to remove the tonsils, or if complications develop after tonsillitis (joint pain, kidney inflammation),<\/li>\n<li>chronic pharyngitis, white patches in people who have taken antibiotics or used inhaled steroids (fungal infection),<\/li>\n<li>public health screening for infectious diseases and testing requested by an occupational health doctor.<\/li>\n<\/ul>\n<h2>How to prepare<\/h2>\n<ul>\n<li>Ideally, attend in the morning, before breakfast and brushing your teeth. For at least 2 hours, avoid eating, drinking, chewing gum, smoking, rinsing your throat or nose, and using lozenges, syrups or sprays.<\/li>\n<li>If possible, have the test before starting antibiotics; if you are already taking them, inform the staff (the result may be falsely negative).<\/li>\n<li>Collection point staff take the swab from both tonsils and the back of the throat; this takes a few seconds and may trigger the gag reflex.<\/li>\n<li>Samples are accepted Monday to Friday; check the opening hours of your chosen collection point.<\/li>\n<\/ul>\n<h2>How to read the result<\/h2>\n<table>\n<tr>\n<th>Result<\/th>\n<th>What it usually means<\/th>\n<\/tr>\n<tr>\n<td>normal throat flora<\/td>\n<td>no disease-causing bacteria; if symptoms are present, the cause is most likely viral and antibiotics are not indicated<\/td>\n<\/tr>\n<tr>\n<td>Streptococcus pyogenes (group A), also groups C or G<\/td>\n<td>streptococcal tonsillitis or carriage; if symptoms are present, an antibiotic based on susceptibility testing, usually penicillin<\/td>\n<\/tr>\n<tr>\n<td>Staphylococcus aureus, Haemophilus, Moraxella and others<\/td>\n<td>often colonisation, not disease; treatment depends on the clinical picture, not the result alone<\/td>\n<\/tr>\n<tr>\n<td>Candida albicans<\/td>\n<td>colonisation in 20\u201350% of healthy adults; treatment only if symptoms of a fungal infection (patches, burning) and risk factors are present<\/td>\n<\/tr>\n<tr>\n<td>antibiotic susceptibility: S (susceptible) \/ R (resistant)<\/td>\n<td>the doctor selects a medicine marked S; the presence of antibiotic susceptibility results does not mean an antibiotic is necessary<\/td>\n<\/tr>\n<\/table>\n<p>A culture result is always interpreted alongside symptoms. Remove dentures just before the sample is taken. Antibiotics taken beforehand may cause a false-negative result.<\/p>\n<\/details>\n<details>\n<summary>Will a culture detect a virus?<\/summary>\n<p>No. A culture grows bacteria and fungi. Around 90% of cases of pharyngitis are caused by viruses, which antibiotics do not treat. If the doctor suspects a viral infection (runny nose, cough, hoarseness, no patches), a culture is usually unnecessary.<\/p>\n<\/details>\n<details>\n<summary>Streptococcus pyogenes has been cultured. What next?<\/summary>\n<p>Group A streptococcus is the main bacterial cause of tonsillitis and requires antibiotics (usually penicillin) to prevent complications: an abscess, rheumatic fever and kidney inflammation. Antibiotic susceptibility testing shows which antibiotics the strain is susceptible to.
